What do I need to have with me to Identify myself?
A Driver’s License or ID issued by a DMV office in any state issued in the last 5 years
Or
A U.S .Passport
Or
Any Employee Card issued by any state, city, county of city in the U.S.
Or
Military ID.
What if I am visiting from Mexico or Canada?
A Driver license issued by any state, local or territory of Mexico or Canada.
Can I have a witness Identity me?
You must have two credible witnesses who know the signer with state approved ID Cards.
Or
One witness known by the Notary.
Can the two witnesses be related to the signer?
At least one witness must not be related in any way to the signer. Moreover, the signer’s attorney in fact (someone acting under power of attorney) health care providers or operator or employees of health care facilities may not serve as a witness.
Can a Mark such as an “X” serve as a signature?
Two Witnesses are required. Both witnesses must sign the document, and one witness must sign their name next to the Mark.
Can minors be notarized?
No. The parents or guardian must sign and be notarized.
